Client Care Reflection
During client care and monitoring, we participated in input and output, vitals monitoring, hand hygiene, purewick catheter, and review. The client was U.S., a 98-year-old male with 81.4 kg and a height of 172.7cm. The client had no known or reported allergies to food or drugs. The client was diagnosed with a right ureteral stone and had been put on acetaminophen by Dr. Argow.
The client had right urinary incontinence and a history of cystic polyps. The client’s past surgical history included a proximal dissection performed in July 2022. The client also had a history of hernia repair. Under medical history, the client reported having stopped taking Brilinta 90mg and aspirin 81mg for approximately one and a half weeks.
The client care was at 8 a.m. for the client’s history of urinary tract infections (UTI) and enterococcus faecalis. The past medical history included hypertension, hypersensitivity lung disease (HLD), community-acquired pneumonia (CAP), and prostate gland hyperplasia. The client presented with flank pain and low-grade fevers and was treated for acute cystitis with Macrobid for 7 days.
The client’s vital signs were a blood pressure of 138/51, HR 65b/min, temperature of 36.4 0C, and SPO2 of 98%. The fall risk was low, and the client had a right anti-tubal peripheral intravenous site. The client also had a Foley catheter due to urinary tract obstruction secondary to kidney stones. He was on a clear diet. Under general assessment, the client was alert and oriented to time, place, and person with no complaints of pain. Systemic examination showed no abnormalities. The client reported no tenderness along the thoracic or lumbar spine. The client’s recommendations included 1(4oz) apple juice, ½ cup yellow-green, 6oz chicken lad, leach fruit ice (lemon), and 1 cup of coffee.
